In reality this is the sort of question that we can only really answer one way (and why we cannot give specific advice).
Officially you should report any change of circumstances as soon as they occur as per your letter, albeit logic may say, to wait to see if the medication does work or not. However having no idea of the timescale that you refer to that it takes for the medication to start to work, if it is a couple of days then it maybe wise to wait, if longer and more importantly if improvement/deterioration occurs then really you know the answer, hence we can only advise to follow the official policy.
