DLA renewal 4 page form?

I always do Jeff1. Learnt this from past experience dealing with these people. I'm trying to adapt most information from previous form i copied to fit the new DLA1A form which is slightly different. The boxes "how often" and "how long" are causing problems. Thanks.

Just a quick question. I have a letter provided by my consultant to support my DLA application. Do i send the original or is it best to copy it and send them the copy?

Thanks for all your help. Received my DLA letter today, same award as before. Only thing is they have not stated an end date this time, my previous award was 3 years. I wonder if this is connected to the coming changes to DLA in 2013 announced?
I returned the DLA580 and a Downloaded copy of the DLA1A form with much detail in.

Hi Vision,

If there is no end date, this means that your award is 'indefinite' ( formerly called life awards).

It is possible that the DLA Unit could contact you at some time in the future, but so long as your mobility and/or care needs don't improve, you should have no problems.

I'd be inclined to send the original by recorded delivery...that way it is a completely transparent submission.

I suppose on the other hand you could indeed send a copy - and state the original is available for viewing if they so wish.

Covered your bases either way.
