Quick update- my review form finally arrived! Ive spent all evening completing it- wow- it's intensive. Have supplementary evidence attached and repeat prescriptions plus a letter of support. Glad I did as much as I could up front writing on a word doc my thoughts - allowed me to transpose this into the review form. Will be sent tomorrow and then I wait some more....
