Well I can't tell you exactly as my medical conditions are only really relatable to me and I know I'm an odd case, but I got him to do a quick break down of what my ailments were and how I had difficulty in taking most medications, which in turn made it hard for them to help me.
I also listed what steps and treatments I'd had down the years and why they hadn't worked, backed up by old and newish letters from previous GP's.
